LIVING WITH LONG TERM CONDITIONS

We believe that it is very important to minimise the effect of living with any long term conditions. This involves regular monitoring of your illness and providing you with all information you need to help you look after yourself. We offer specific appointments either in specific clinics or ordinary surgeries to review the following illnesses:

• hypertension (high blood pressure)
• coronary artery disease (angina and heart attacks)
• stroke
• diabetes
• asthma
• chronic obstructive airways disease
• chronic kidney disease
• epilepsy

We also believe that it is very important that patients with long-standing mental health problems or severe learning disabilities should be able to make full use of our services so we offer a full annual health check to these patients.
