What is My Practice Online? It is a secure, confidential internet service that allows you to book an appointment, request a repeat prescription or notify us if you change your address. You will be able to see free apppointment slots and choose the one that suits you best. You can cancel appointments if you are unable to keep them. To ensure that the information remains confidential, you will have to use a password to enter the secure EMIS Access website. You can also view a summary of your medical record kept by the practice. This needs a separate login that you can obtain from the practice. EMIS Ltd is the company that supplies our practice computer system, including the web-based EMIS Access. EMIS have recently changed the web design and the web address of EMIS Access. If you still prefer the old version you can still use it. Just click here to login.
Some restrictions on your use of the online appointment service Please be aware that it is possible to cancel appointments online, but please do not cancel Heartbeat or Diabetes clinic appointments this way. We rely on the clinic appointment lists to send out invitations and if you do cancel an appointment this way, you may not get another invitation automatically. If you want to cancel or change a Heartbeat of Diabetes clinic appointment please telephone reception at Bentham surgery. Also please do not book appoinments for medicals online. They need more than one appointment slot. Please call reception to make an appointment that is long enough for the type of medical examination that that you need. You cannot use the system to request home visits or to contact the practice in an emergency.
How do I register to use the service? To begin, you must register in person at reception. We will give you all the details you need to create your account and password to use immediately but you can then change it so that nobody, including the practice can access your information without your permission. To ensure confidentiality, we will not let one person sign up on another person’s behalf and we restrict access to people over the age of 16. We also need proof of identity to be sure that we only grant access to the person with right of access to the record. Acceptable proof of identity is one of the following (originals only): birth certificate; passport; photo card driving licence; national identity card; Home Office identity card. Or two of the following bearing the patient’s name and dated in the last three months and showing the address being registered (originals only): statement from a financial institution (bank, building society, credit card or mortgage lender); bill from a utility company (gas, electric, water or fixed line telephone provider); council tax bill; letter confirming entitlement to housing benefit, council tax benefit or tax credits; letter from the Home Office in relation to an application for asylum or appeal; letter from your educational establishment that you are currently attending.

Medical Records Viewer A special extension of the EMIS Access system allows you to see a summary of your health record held by the practice. You can find out more about it by downloading our leaflet about it here (125KDB, last updated 26/11/09). The leaflet includes a copy of the consent form that we ask you to complete bfore we can switch on access to your medical record. |
