HOW TO REGISTER WITH THE PRACTICE

We welcome new patients to the practice if you live within the practice area. We hope there is enough information on our website to explain how we work and the services we offer. You can Download a copy of our Practice Leaflet.

You can register at either of our surgeries in Bentham or Ingleton. One person can register for the whole family but it is essential to bring proof of identity for everyone over the age of 15 who wants to register with us. This is in line with NHS policy. If you would like to read more about registering with a GP practice there is lots of information on the NHS Choices website.

 

Registration form

If you do not have your medical card please fill in the NHS form called the GSM1 before you come to the surgery. We will need the address at which you were last registered with a GP. The post code is especially important for us to complete your registration. You can download a copy of the GMS1 here.

If you want to register with the practice at the same time as your first appointment with a doctor or a nurse, please arrive ten minutes earlier to complete your registration at reception.

Dispensary

If you live more than a mile from one of the chemist's shops in High Bentham, Ingleton or Caton, you can register to collect your prescribed medication from the practice.

This means you can pick up your medication at the surgery or we can arrange for your medication to be delivered to pick up points in all the main villages in the practice area.

Of course if you register with us as a dispensing patient, you can still use any chemist to make up a prescription from us.

New patient medical check up

We recvommend that every new patient has a health check at the surgery. If you have any ongoing health problems it helps us tounderstand what they are and advise you how we can help. If you are well, then we can advise on all aspects of healthy living. The receptionist will be able to tell you more and make appointments for you and your family with one of our practice nurses.
