Primary care Networks were introduced as part of an NHS long term plan with the ambition of creating joined up and personalised care with a strong emphasis on collaborative working to encourage efficiency , service delivery, and shared learning.

The PCN provides our practice with skilled professionals that help our patients through a direct route and enables an efficient approach to problems. For example, The PCN have a shared first contact physiotherapist working across all 3 practices, additional pharmacists, counsellors, care navigators and social prescribers. These member of staff are playing an integral part in keeping our surgery, as well as our neighbouring surgeries, running a service for our patients.
